Application Requirements

Application for the Berlex Scholar Award in Clinical Research shall include the following:

Application Cover Form – Download and complete the Application Cover Form. To download the cover form with:

Internet Explorer - Right click the link above and select "Save Target As..."
Netscape/Other - Click the link above and choose "Save this file to disk"

Curriculum Vitae – including basic personal data, educational background, honors, past and present positions.

Letter of Recommendation – from the Chairman of the Department, a senior faculty member of the facility where the applicant is currently affiliated, and the Director of the laboratory in which the investigation is to be conducted. The letter from the Chairman should indicate agreement to enable at least 50% of the candidate's time to be devoted to the research activity.

Statement of Career Goals – one page

Research Proposal – presented in NIH format not to exceed six typewritten pages. Additional supporting information may be provided in addendum form if deemed essential. A one-page budget summary outlining proposed expenditures should be included.

Note: It is the policy of the Berlex Foundation that no indirect institutional costs are to be deducted from the award. Inclusion of indirect costs or institutional overhead in budget will be disallowed.

Summary of Other Applications – Any concurrent applications for funding of the same research project should be identified in summary form. In the interest of fair distribution of support, and to avoid duplicate funding from multiple sources, the final award will be predicated on candidate's agreement not to accept additional funding for stated research within the award year.

All required documents must be submitted at one time. No partial submissions will be considered.

Application Deadline: Applications must be received by: October 1, 2007. Applicants selected for the award will be notified no later that December 15, 2007.
